Before submitting this form, please read the following information carefully. If you have any questions, please feel free to discuss them with me prior to your first consultation.
 

I understand that Nancy Buono, BFCP, The Inner Gardener LLC, is not a licensed physician, nurse, psychologist, or other licensed medical practitioner in any U.S. state. I further understand that she is a Bach Flower Certified Practitioner, professional wellness educator and consultant, member of the Bach Flower Practitioner Alliance, consultant of Natural Health and Life Science, a religious science of health, and a Personal Development Mentor.

I understand and acknowledge that consultations, Bach flower remedies, and any other wellness, educational, or personal development modalities discussed or recommended do not diagnose, prescribe for, prevent, or treat disease. The diagnosis and treatment of disease are solely within the scope of licensed medical professionals. If I know or suspect that I may have a condition requiring medical attention, I understand that I should seek the care of an appropriately licensed healthcare provider. I understand that consultations are educational in nature and are intended to support personal growth, self-awareness, and reflection regarding my spiritual, mental, and emotional well-being. Discussions are client-centered and guided by my own concerns and goals.

I understand that Bach flower remedies may be used alongside orthodox, complementary, or alternative approaches to wellness, and that participation in these consultations is entirely voluntary.

I understand that my personal information and records will be kept confidential except:

• at my request,
• as required by law or court order,
• or if consultation with another practitioner is needed on my behalf or for educational purposes, in which case my identity will remain confidential.

I understand that all information and recommendations shared during consultations are offered for educational purposes only and are to be applied at my own discretion and responsibility.

By submitting this form, I acknowledge that I have read and understood this disclosure, understand the limitations of these services, and voluntarily choose to participate in consultations with Nancy Buono, The Inner Gardener LLC. I accept full responsibility for my own healthcare decisions and hold Nancy Buono and The Inner Gardener LLC harmless regarding my choices and participation.
